Wiltshire Brazilian Jiu Jitsu Academy – Membership Agreement & Waiver

I confirm that the information I have provided is true and accurate to the best of my knowledge. I understand that martial arts training is a physical activity, and while every effort is made to provide a safe and supportive environment, I accept that there is a natural risk of injury. I take personal responsibility for my health and wellbeing (or, where applicable, for the wellbeing of my child or the person under my care) during training. I agree not to hold Wiltshire Brazilian Jiu Jitsu Academy or its instructors responsible for any injuries or for loss or damage to personal property.

I agree to follow the rules and code of conduct set by Wiltshire Brazilian Jiu Jitsu Academy, helping to create a safe, respectful, and positive training atmosphere for all. I confirm that I (or my child/dependant) am physically and mentally able to participate in martial arts training.

I understand that an annual membership licence fee is required to attend classes and remain an active member. I also acknowledge that Wiltshire Brazilian Jiu Jitsu Academy reserves the right to decline, suspend, or cancel membership if necessary to protect the safety and wellbeing of the academy and its members.

For members under 18 years of age or vulnerable adults, this agreement must be completed and signed by a parent, guardian, or responsible carer.
